To appear in the list, menu entries need to specify icon48=[icon] and
section=[section]. It is possible just to specify icon48=[icon that
doesn't exist]

That's right. Did you *really* want every single menu item in the list?
When developers package the icons, they'll appear. (Nils & I might be
bugging developers about that soon, with patches etc. if there is still
nothing happening).
